Description:
Channel 415 is frequently noisy or drops out altogether.  Seems to be a continuation of 
the problem addressed in DQR D050920.1.  Instrument operated like this until 1/10/2006 when 
a catastrophic failure occurred.  Unknown if this failure is related to the 415 issue.  
Instrument was pulled for repair.
