Description:
Channel 940 drops out or is noisy at times.  First noticed on 24 Aug 2006, it is "dead" 
until 20 Sept 2006.  Other dates where problems are noted:

30 Jan 2007 - 2 Feb 2007
1 April 2007 
12 Aug 2007 - current

There could be other times when 940 was exhibiting problems that were not caught by the 
mentor, so this DQR is being written to cover the entire period.
